We are concerned about your dental care and want to ensure you that it is performed in the most responsible manner. In order to assist you with the investment in your dental health, we have outlined our payment options.
• Drs. Fertik and Broderick accept all major credit cards, personal checks and cash as forms of payment.
• For your initial exam, we request payment on that day as the majority of dental insurances will not cover the first consultation visit.
• For other recommended treatment, our Financial Coordinator will discuss payment options with you prior to scheduling that treatment. Options generally include:
• Assisting you in filing for dental benefits and we ask that an estimated percent of our fee be paid at time of treatment until more specific benefits are known.
• Although we do participate as a contracted provider with several insurance companies, we do accept direct assignments of benefits and we will gladly assist you in maximizing your insurances. Your anticipated copayment will be due at time of service. In order to do this for you, we ask that you provide us with your dental insurance information at your first visit.
• Please remember that your dental insurance policy is an agreement between you and your insurance company and, ultimately, you are financially responsible for all services rendered.

Our staff will do their utmost to process pretreatment estimates and inform you of anticipated insurance benefits available for your periodontal treatment. In addition, we are also participating in CareCredit revolving healthcare credit cards. Please ask our administrative staff for information regarding this alternative for treatment not covered by your dental insurance.
